Please download the Mobile Lactation Consultant app on mobile to complete your registration forms.
**NOTE: Forms must be complete before your appointment can begin. Please arrive at least 15 minutes early if you need to complete the information in our office. We run on time, and the duration of appointments will not be extended.

Be sure to bring the following:
✔ try to have your baby ready to eat 15-30 minutes into your appointment time
✔ bring your breast pump
✔ bring any supplemental milk or formula you may need
✔ any other breastfeeding supplies that you currently use or desire to learn about
**Please notify us ASAP if you or your partner are feeling unwell within 24 hours before your appointment.**
Cancellation Policy
Please give us 24 hours to cancel or reschedule your appointment. There is a $50 cancellation fee / $25 rescheduling fee without 24 hours notice.
This policy is in place to ensure we can see families in need on a timely basis. We understand things come up, but please respect the needs of all mothers and babies, as well as your lactation consultant’s time, as we do block 2-3 hours per appointment.
If you need to change your appointment, please Call or text us as soon as possible. Phones are monitored after hours and over the weekend. Cancellation/rescheduling fees are not reimbursable by insurance.
