Please use a MOBILE DEVICE to download the Mobile Lactation Consultant app and complete your forms prior to your appointment.

**Note: this is a mobile app and works best on a phone, iPad or tablet.**
Your appointment cannot begin until these forms are complete. Please be aware that completing them at the start of your appointment will take up some of your allotted time. Please arrive at least 15 minutes early if you need to complete the information in our office.


How To Log Into the Client Portal:

  1. Download the Mobile Lactation Consultant App
  2. Type “My Pure Delivery” in the ‘Practice Name’ field.
  3. Leave the password fields blank. **DO NOT enter an email address or password.** 
  4. Click “Sign On”
Cancellation Policy
Please give us 24 hours to cancel or reschedule your appointment. There is a $50 cancellation fee / $25 rescheduling fee without 24 hours notice.   
This policy is in place to ensure we can see families in need on a timely basis. We understand things come up, but please respect the needs of all mothers and babies, as well as your lactation consultant’s time, as we do block 2-3 hours per appointment.
If you need to change your appointment, please call or text us as soon as possible. Phones are monitored after hours and over the weekend. Cancellation/rescheduling fees are not reimbursable by insurance.
